Sage

Sage, the largest program within the Sage Screening Program family, is a breast and cervical cancer screening program. For eligible women, Sage provides free office visits for breast and cervical exams, as well as a screening mammogram and Pap smears. If one of your screening tests shows a problem, Sage covers many diagnostic services and can often cover treatment, if needed. Sage partners with over 400 clinics throughout the state. Sage Scopes

Sage Scopes, the newest addition to the Sage Program family, is a free colorectal cancer screening program. It provides free colonoscopies, as well as free prep materials and select related services to eligible men and women. While Sage Scopes accepts patients from across the state, it currently has a waiting list and only has screening sites in the Twin Cities and in Austin. Please note that Sage Scopes has different eligibility guidelines than other Sage programs, and eligibility guidelines differ between screening sites. Call 1-888-6HEALTH to see if you’re eligible.

Sage Plus

SagePlus is a heart-health program for women who participate in Sage. SagePlus provides eligible women with free services including tests for blood pressure, cholesterol, glucose and body mass index, life-style coaching on topics such as diet, exercise and smoking cessation, as well as a monthly newsletter and fun programs with prizes to encourage you to exercise more and eat healthier. Ask about SagePlus when you’re scheduling an appointment with Sage. SagePlus services are available at a limited number of clinics throughout the state

The Sage Programs are working to connect Minnesotans interested in quitting smoking with the QUITPLAN Helpline.

Innovation

The Innovations grant is a collaborative project between the Minnesota Department of Health (MDH) and the Minnesota Department of Human Services (DHS) that aims to increase breast and colorectal cancer screening rates statewide among enrollees of Minnesota’s State Health Care Programs (SHCP), which includes Medicaid and MinnesotaCare. Highly-effective direct mail (DM) materials coupled with small financial incentives, and patient navigation services, are being used to promote breast and /or colorectal cancer screening among age- and gender-appropriate SHCP enrollees overdue for screening. The MDH is working with DHS to sustain this effort and expand it to other organizations, both locally and nationally.
